Please be prepared to provide insurance information when you check into the clinic. Co-pays, as dictated by your insurance plan is due at the time of service provided. We accept major credit cards, cash, and money orders.

Accepting New Patients
Prescriptions & Refills
New prescriptions usually require an office visit. For medication refills, please contact your pharmacy and have them send a request to the office. For medications requiring a written script, contact the office during business hours. In either case, allow 3 business days for the refills to be processed.
